This Privacy Policy explains how Apex Wellness Clinic (“Apex Wellness,” “we,” “us,” or “our”) collects, uses, discloses, and protects information when you visit our websites, submit a form, or otherwise interact with us. Please read it alongside our Terms of Service.

1. Information we collect

Information you provide

• Contact and intake details you submit through our forms — name, email address, phone number, the U.S. state you’re in, and the type of service you’re interested in.
• Communications you send us by phone, email, text, or chat, including any information you choose to share about your health goals.

Information collected automatically

• Device and usage data — IP address, browser type, device identifiers, pages viewed, referring URLs, and interactions with the page.
• Cookies and similar technologies — including advertising and analytics identifiers. See “Cookies and tracking” below.
• Advertising click identifiers — for example, Meta’s click identifier (fbclid) and campaign parameters (UTM tags) passed in the web address when you arrive from an ad.

We do not intentionally collect sensitive health details through our advertising pixels, and any symptom selections you make in interactive tools on our site stay in your browser and are not written into the page address or transmitted to advertising platforms.

2. How we use your information

• To respond to your inquiries, schedule your free consultation, and provide the services you request;
• To operate, maintain, secure, and improve our websites and services;
• To communicate with you about appointments, offers, and updates (you can opt out of marketing at any time);
• To measure and improve our advertising, including matching conversions to the ads you saw;
• To detect and prevent fraud, spam, and abuse; and
• To comply with legal obligations and enforce our agreements.

3. Cookies and tracking technologies

We and our providers use cookies, pixels, and similar technologies to run the site, remember your preferences, measure performance, and support advertising. When you arrive on our site, these advertising and analytics technologies — including the Meta Pixel — load automatically and begin collecting the information described in this Policy. We use the Meta Pixel and Meta’s Conversions API to understand which ads lead to inquiries. These tools may set cookies and receive limited information to measure conversions. We do not send the health details you enter into our forms (such as symptoms or the service you select) to advertising platforms.

You can control or refuse cookies through your browser settings and limit ad tracking through your device or platform settings — including the browser-level opt-out signals we honor where required by law (see “Your privacy rights”). 4. How we share information

We do not sell your personal information for money. We share information only as described here:

• Service providers who work on our behalf — for example, our CRM and scheduling platform (GoHighLevel / LeadConnector), hosting and analytics providers, and our laboratory partner (LabCorp) — under contracts that limit their use of your information.
• Advertising platforms such as Meta, to measure and optimize campaigns as described above. Depending on your state, some ad-related sharing may be considered a “sale” or “sharing” of personal information, which you can opt out of (see “Your rights”).
• Legal and safety — when required by law, to respond to legal process, or to protect the rights, safety, and property of Apex Wellness, our patients, or others.
• Business transfers — in connection with a merger, acquisition, or sale of assets, subject to this Policy.

5. Data retention

We keep personal information for as long as needed to fulfill the purposes described in this Policy, to comply with our legal and recordkeeping obligations, resolve disputes, and enforce our agreements. Health records are retained according to applicable medical-records retention laws.

6. How we protect your information

We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit and access controls. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

7. Your privacy rights

Depending on where you live, you may have rights to: access the personal information we hold about you; correct or delete it; obtain a copy in a portable format; and opt out of targeted advertising, “sales,” or “sharing” of your personal information. We will not discriminate against you for exercising these rights.

“Do Not Track.” Some browsers offer a Do-Not-Track signal. Because there is no common industry standard, we currently respond to recognized opt-out preference signals (such as Global Privacy Control) where required by law.

8. Children’s privacy

Our services are intended for adults 18 and older. We do not knowingly collect personal information from children. If you believe a child has provided us information, contact us and we will delete it.

9. Third-party links

Our site may link to third-party websites and services we do not control. Their privacy practices are governed by their own policies, and we encourage you to review them.

10. Changes to this Policy

We may update this Policy from time to time. When we do, we will revise the “Last updated” date above, and material changes may be communicated through the site or by other means.
